Making a referral to IMCA

 

Who can make a referral?

Health organisations and local authorities can refer people to the service by calling us on 01449 771590. The general public cannot refer people to us directly, but you can request a professional to consider making a referral if it is appropriate.

Anyone needing to refer someone will need to fill in and return the referral form and the Functional Test of Capacity Form (which is included as part of the referral form) which must be signed by a decision maker.

This can be emailed to us on advice@optua.org.uk or faxed to 01449 770135 but we must also receive a hard copy of the form in the post for our records.
